Obverse – The center of the coin features the image of Her Majesty Queen Sirikit, facing right, wearing the Diamond Crown and Diamond Earrings, Thai dress with an embroidered shoulder strap, and wearing the sash and necklace of the Most Illustrious Order of the Chakri Dynasty. The left shoulder is decorated with the First Class Chula Chom Klao Emblem and the Ratana Phorn Medal of King Rama IX, First Class. Within the right rim of the coin is the inscription “Her Majesty Queen Sirikit” and the left rim is the inscription “Phra Borommarajini”.
Reverse – The center of the coin features the emblem of the celebration of Her Majesty Queen Sirikit's 6th Cycle Birthday Anniversary on 12 August 2004. Within the right edge of the coin is the denomination. On the left is the word "Baht". Above is the text "Maha Mongkol 6th Cycle Birthday Anniversary". Below is the text "Thailand 12 August 2004".
